Major bone grafting is necessary if the jaw has severe damage from trauma, prior surgery, or congenital defects. When the jawbone has severely atrophied or resorbed, major bone grafting involves repairing the jaw with bone tissue harvested from the patient’s body. Most major bone grafting procedures are performed in a hospital setting and involve harvesting bone tissue from a patient’s hip or tibia bone.

Minor Bone Grafting: Ridge Expansion, Sinus Lift, and Onlay Bone Grafts

Minor bone grafting procedures are typical for patients pursuing dental implant surgery. The most common minor grafting procedures include ridge expansions, sinus lifts, and onlay grafts. In these procedures, bone tissue may be harvested from the patient’s own body or a tissue bank. Unlike major bone grafting procedures, minor grafting is an outpatient procedure at our Park Ridge office.

Ridge Expansion and Sinus Lift

When the ridge of the jaw is too thin to support dental implants, Dr. Sisto might recommend a ridge expansion procedure. Ridge expansion increases the width and height of the jawbone where the implant will be placed. A ridge expansion procedure begins several months before implant placement, allowing the bone grafting material to mature.

A sinus lift or sinus augmentation is a minor bone grafting procedure to make the jawbone suitable for implants near the premolar and molar teeth. A sinus lift places a bone graft underneath the Schneiderian membrane to increase the bone tissue in a region of the upper jaw called your posterior maxilla.

Major Bone Grafting

Bone grafting can repair implant sites with inadequate bone structure due to previous extractions, gum disease or injuries. The bone is either obtained from a tissue bank or your own bone is taken from the jaw, hip or tibia (below the knee). Sinus bone grafts are also performed to replace bone in the posterior upper jaw. In addition, special membranes may be utilized that dissolve under the gum and protect the bone graft and encourage bone regeneration. This is called guided bone regeneration or guided tissue regeneration.

Major bone grafts are typically performed to repair defects of the jaws. These defects may arise as a result of traumatic injuries, tumor surgery, or congenital defects. Large defects are repaired using the patients own bone. This bone is harvested from a number of different sites depending on the size of the defect. The hip (iliac crest) and lateral knee (tibia), are common donor sites. These procedures are routinely performed in an operating room and require a hospital stay.

Ridge Expansion

In severe cases, the ridge has been reabsorbed and a bone graft is placed to increase ridge height and/or width. This is a technique used to restore the lost bone dimension when the jaw ridge gets too thin to place conventional implants. In this procedure, the bony ridge of the jaw is literally expanded by mechanical means. Bone graft material can be placed and matured for a few months before placing the implant.
